Thanks for your inquiry.

The California Department of Justice provides an automated service for criminal history background checks that may be required as a condition of employment, licensing, certification, foreign adoptions or VISA/Immigration clearances.

You must get instructions and the forms required for fingerprinting services from the agency requiring your criminal history background check.

You should download form download form BCII 8016 which is a request for record clearance from the DOJ for Visa/Immigration purposes.

Applicants who are living outside California/United States must submit manual fingerprint cards - Form FD258 - I am not sure if you can download this form from the internet. If you are having difficulty in obtaining a blank fingerprint card, please contact the Record Review Unit at (916) 227-3849 or maybe the police authorities in Guam can provide you this form.

After that you should contact your local law enforcement agency for fingerprinting services. If you are outside US, then you can use the FBI finger print card or international finger print services.

PLEASE NOTE: Your fingerprint card must contain your full name, date of birth, sex and return mailing address. Manual submissions must be accompanied by either personal check drawn on a U.S. bank, money order or certified check. The check or money order must be made payable to the California Department of Justice. Mail your application, fingerprint card and processing fee to:
